INSTRUCTIONS
After clicking on the link to the Certification Packet (below),
click on <OPEN> when the first dialog box appears.
Note: this may take a few
moments, as the Certification Packet is 5 megabytes.
After the document downloads to your local computer,
click on <FILE>, then click on <SAVE AS> and save
the Certification Packet as a WORD DOCUMENT (*.doc) to a convenient
location on the local computer.
IMPORTANT:
Do not save as a Template; if saved
as a Template, all input will be lost
each time the file is closed.
Saved as a WORD DOCUMENT, The Certification Application behaves
like any Word document. As each section (grey box) is completed,
simply click SAVE. Applicants can return to the form as
often as needed to
complete additional sections.
The document is password protected to allow only the grey boxes
to be filled in with text.
